They ban if you manually input an IP that you were not assigned from the DHCP server, a VPN doesn’t actually change the IP that you are assigned, it only changes the IP that the rest of the web sees. The VPN is a tunnel, you still maintain your own cox IP on your computer and this is the IP they care about, it’s just that all info is passed through another server before it makes it to your destination and then all info is passed back through that server.

Cox has only sent me a single notification, and it was for a show I’ve never watched, let alone pirated.

Even though I now do all my downloading via a VPN, I can’t help but wonder if Cox will continue to penalize me for downloading things I’ve never seen.

And the government can tell people what they’re allowed to drive on the highway, how fast they can drive, when to wear seat belts, and pretty much everything else. Hence my likeness to the highway, you may pay for it, but someone else makes the rules for it.
